Traumatic Heart Injury - A review of 14 cases




Abstract
From January 1982 to December 1989, 14 cases of cardiac injured patients were operated. There were four blunt cardiac trauma and ten penetrating cardiac injuries.
The results were as follows,
1) The sites of injury in penetrating trauma were six on right ventricle, three on left ventricle and one on right atrium, and in blunt, three were left atrial appendage ruptures, and one was left atrium and ventricular rupture.
2) Cardiac tamponade was noted in 10 cases, and elevated central venous. pressure over 13 cm H2O was in 10 cases.
3) At admission, all patients of penetrating cardiac wounds showed shock state, intial systolic blood pressure were below 90mmHg,
4) Patients had operated within six hours, the mean was 158 minutes, in the cases of penetrating heart wounds.
5) Combined injured sites were as follow; hemothorax in seven, 1ung injuries in four, rutured internal mammary artery in four, stemal fracture in four, and delayed spleen rupture in one case.
6) Extra corporeal circulation was used in one case for closure of aortic-right ventricular shunt, after 4 days of primary closure of right ventrcular wall injury.
7) There were two operative deathes, one was m blunt left atrial and ventrcular rupture, and the other was in right ventricular stab wound with arrested heart.
8) Postoperative complications were two of pericarditis, and one of posttraumatic psycosis. With the brief review of the literatures, we report the cases.
